Andreas Rheinhardt 790f793844 avutil/common: Don't auto-include mem.h 
There are lots of files that don't need it: The number of object
files that actually need it went down from 2011 to 884 here.

Keep it for external users in order to not cause breakages.

Also improve the other headers a bit while just at it.

Anton Khirnov 1e7d2007c3 all: use designated initializers for AVOption.unit 
Makes it robust against adding fields before it, which will be useful in
following commits.

Majority of the patch generated by the following Coccinelle script:

identifier arr_name;
initializer list il;
initializer list[8] il1;
expression tail;

AVOption arr_name[] = { il, { il1,
- tail
+ .unit = tail
}, ...  };

with some manual changes, as the script:
* has trouble with options defined inside macros
* sometimes does not handle options under an #else branch
* sometimes swallows whitespace

Andreas Rheinhardt e05d3c1a16 avdevice/caca: Allow to list multiple dither option types at once 
This can be achieved by using AV_OPT_TYPE_FLAGS instead of
AV_OPT_TYPE_STRING. It also avoids strcmp() when accessing
the option.

Anton Khirnov 08bebeb1be Revert "all: Don't set AVClass.item_name to its default value" 
Some callers assume that item_name is always set, so this may be
considered an API break.

Andreas Rheinhardt 59c9dc82f4 avformat/avformat: Move AVOutputFormat internals out of public header 
This commit does for AVOutputFormat what commit
20f9727018 did for AVCodec:
It adds a new type FFOutputFormat, moves all the internals
of AVOutputFormat to it and adds a now reduced AVOutputFormat
as first member.

This does not affect/improve extensibility of both public
or private fields for muxers (it is still a mess due to lavd).

Stefano Sabatini c0b91348fe lavd/caca: create display *after* checking dither options 
Avoid fastidious creation and immediate destroy of the display in case
one of the dither options fails to be set.

Also add NULL checks in write_trailer(), which are now necessary since
the function can be called when not all the components have been
initialized.

Stefano Sabatini fa71471cef lavd/caca: rename "colors" option to "color"
